The transition services program helps guests take critical next steps and move forward in their lives through goal setting, weekly meetings, support and accountability.
Transition Services staff help our community members navigate complex social services systems and advocate on their behalf. Our trained case workers help with problem solving and provide support in overcoming a variety of barriers.
This support looks different for each person, but often includes practical assistance with getting ID, accessing financial benefits, or connecting to resources like doctors and mental health or addictions treatment. Our staff also help with finding housing, in-home supports for preventing eviction, and legal issues.
As part of Transition Services we offer a variety of field trips for our guests. From overnight camping trips to artistic and cultural excursions, these trips are a real source of joy and hope for our community.
Many people experiencing homelessness lack proper personal identification which is essential in helping them get health care or a job.
At Siloam Mission, hope begins with a meal. It is by first filling basic needs that we can help our guests begin to look to the future.
